PERSONAL DATA CLAUSE
INFORMATION OBLIGATION REGARDING THE PRINCIPLES OF PERSONAL DATA PROCESSING
of the Academy of Silesia with its registered office in Katowice
Pursuant to Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation – GDPR), applicable as of 25 May 2018, we hereby provide information concerning the processing of your personal data by the Data Controller.

- The Data Controller processes the following categories of personal data:
first name, last name, user identifier within the Data Controller’s website, correspondence address, e-mail address, telephone number, and the IP address of the device used. - Your personal data are processed on the basis of:
- necessity for the performance of a contract to which you are a party or in order to take steps prior to entering into a contract (Article 6(1)(b) GDPR);
- necessity for the purposes of the legitimate interests pursued by the Data Controller or by a third party, including ensuring service security, conducting statistical analyses, and carrying out marketing and promotional activities of the Data Controller’s own services (Article 6(1)(f) GDPR);
- your voluntary consent (Article 6(1)(a) GDPR).
- Recipients of your personal data include entities providing IT system services and technical services to the Data Controller.
- Your personal data will be processed for the period during which the Data Controller or another processing entity holds one of the legal bases for processing, in accordance with the GDPR.
- You have the right to:
- obtain information regarding the processing of your personal data, including information on the categories of data processed and possible recipients;
- request rectification of inaccurate personal data or completion of incomplete data;
- request erasure or restriction of processing of personal data, provided that the statutory conditions for such requests are met;
- data portability, including receiving personal data from the Data Controller in a structured, commonly used format enabling transmission to another controller;
- lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office, 2 Stawki Street, 00-193 Warsaw, Poland, if you believe that your personal data are processed unlawfully.

- Providing personal data is voluntary; however, failure to provide data marked as necessary for the provision of services will prevent such services from being delivered.
